PROSPECTIVE CHAPERONES:
Parents are reminded that they must fill out a “CORI” form before they may be permitted to accompany students on a class field trip.
CORI REQUIREMENTS:
This year, new requirements have been established by the Criminal Board of Investigation and the Archdiocese of Boston. Even if you have previously filled a form you must do it again. Every new school year demands a new form. These guidelines will be strictly enforced by our school personnel.
CORI REPORT PROCEDURE:
All adults planning to be with our students for any reason must complete and sign a CORI form in the presence of an authorized member of our school. We no longer can send forms home to be completed. In addition, when you register, you must present a picture ID. In order to expedite this process you may sign up in the school office. Without this form on file you will not be able to volunteer in school or chaperone any field trips etc.
