Dear Parent/Guardian,
The following screenings will be conducted throughout the 2008-9 school year. Specific dates will be announced at a later date.
VISION GRADES K1-5, 7
HEARING GRADES 1-5
HEIGHTS/WEIGHTS GRADES 1, 4, 7
POSTURAL SCREENING GRADES 5-8
If you do not wish your child to be screened for Vision/Hearing/Postural by the nurse at school, please send a note to the nurse as soon as possible with a copy of your child’s current physical exam.
