PROSPECTIVE CHAPERONES:

Parents are reminded that they must fill out a “CORI” form before they may be permitted to accompany students on a class field trip.

CORI REQUIREMENTS:  This year, new requirements have been Established by the Criminal Board of Investigation and the Archdiocese of Boston.  Even if you have previously filled a form you must do it again.  Every new school year demands a new form.  These guidelines will be strictly enforced by our school personnel.

CORI REPORT PROCEDURE:  All adults planning to be with our students for any reason must complete and sign a CORI form in the presence of an authorized member of our school.  We no longer can send forms home to be completed.  When you register, you must present a picture ID as well as a copy to be attached to the CORI form.  In order to expedite this process you may sign up on Friday, September 18th at the General Parent/Teacher Meeting.  Without this form on file you will not be able to volunteer in school or chaperone any field trips etc.
